Massage Wellness and Liability Form

If you are a new client, please complete this form and bring it along to your first session. It includes a medical history form as well as a standard release of liability for our massage therapy work together.

Cancellation Policy and Credit Card Authorization Form

This form states the cancellation policy and has space to provide the credit card information required to hold an appointment. Please fill this out and send via email before your appointment if you do not already have a card on file.

Minor Liability Release Form

This form allows a parent or legal guardian to give permission to perform massage on a minor client.

Prenatal Massage Release Form

This is a liability release for clients who are pregnant and informs the benefits of prenatal massage.
